Who We Are

CPCN Burleson Medical Clinic is a faith-based, free medical clinic that provides healthcare for the uninsured of Burleson and surrounding areas. We are dedicated to providing high quality medical care in a kind and compassionate environment. Our motivation is to do our part in serving the community.  

We are an all-volunteer organization of doctors, nurses, accountants, pharmacists, social workers, counselors, allied health providers and lay people. Together we are bringing much needed healthcare to our neighbors who are uninsured. We provide free primary care, specialty care (currently gynecology and pediatrics), reduced cost labs, imaging, and prescriptions.

Who is Eligible?

  • Uninsured

  • Meet financial guidelines: at or below 250% federal poverty level

 

  • Live within Johnson County and surrounding area

  • Call for screening and make an appointment 

  • Come to your appointment with required documents and complete paperwork

    • ​Photo ID​​

    • Proof of household income: include all persons living in the home who have a source of income. 

      • Acceptable documents: current pay stubs, previous year's tax form, SSI/SSDI statement, Food Stamp Statement​

      • If you are homeless and staying in a shelter, provide a letter from the shelter

      • If you are temporarily residing with someone, provide a letter from the primary resident

    • Proof of residency

      • Acceptable documents: utility stub or recent mail with your name and current address. ​
